Scalp reconstruction following resection of extensive squamous cell carcinoma complicated by myiasis: a case report

Abstract

Introduction: Oncologic reconstruction of the scalp represents a surgical challenge due to its anatomical complexity and the need to preserve both function and aesthetic outcomes. Objective: To describe the surgical management of an extensive squamous cell carcinoma of the occipital scalp complicated by myiasis. Case presentation: We report the case of a patient with a long-standing squamous cell carcinoma located in the occipital region of the scalp, secondarily infected with larvae. After oncologic resection with tumor-free margins, a defect larger than 15 cm² was identified. Reconstruction was performed in two surgical stages using an occipital rotational flap complemented with split-thickness skin grafts. Postoperative evolution was favorable, with adequate integration of the reconstructed tissues and no complications during outpatient follow-up.


Conclusion: Management of extensive scalp squamous cell carcinomas complicated by myiasis requires a specialized surgical approach. Reconstruction using flaps and skin grafts represents an effective option for complex defects.
